Key points for EVAR: managing short iliac landing zones.
61 yo man, coronary stents and FE<60%, PCOD. BMI>30. 5.5 infrarenal AAA, 2,5 cm straight neck, 24mm , but only 2,2 cm right common iliac landing zone, 13mm diamet and 3,3 cm left iliac length. Careful planning double blinded with my friend from W. L. Gore & Associates, Quintin Dominguez Martin. Excluder stentgraft is one of my favorite choices when I have to be very accurate in deployment and 5 mm cares, and in my opinion the most comfortable to the anatomy endoprothesis. I deployed in ballerina intentionally to facilitate leg canulation based and accuracy in distal right iliac sealing. Important type Ib endoleak observed after right extension deployed corrected after ballooning with Relliant. Final control no leak and result as was detailed in planning. PEVAR under local anesthesia, total time 70 mins. Discharged day 1 after procedure. Again, spending time in planning and sizing is key for successful Endovascular procedures.
